GB Galvanizing Service Pty Ltd faces two workplace safety charges following the death of a 53-year-old worker who was crushed by steel beams in March 2022.

The company faces two charges under section 21(1) of the Occupational Health and Safety Act over an alleged failure to provide and maintain a working environment that was safe and without risks to health.

WorkSafe alleges GB Galvanizing breached regulation 115(3)(b) of the Occupational Health and Safety Regulations by failing to ensure lifted loads were not suspended over, or moved over, a person.

WorkSafe also alleges the company breached section 21(2)(a) of the OHS Act by failing to provide or maintain a safe system of work to eliminate or reduce the risk of serious injury or death from falling loads.

The worker died when lifting equipment he was operating detached, causing steel beams weighing several tonnes to fall onto him.

A filing hearing is listed at Melbourne Magistrates' Court for 28 March 2024.
