Formwork must be planned and managed as a temporary structure, not treated as a stack of reusable components. Before erection starts, confirm the design, loads, foundations, sequence, access, fall controls, responsibilities and inspection points. Keep the system within its design during erection, the pour, curing and stripping.
This guide gives practical orientation for Australian builders, subcontractors, supervisors and tradies. Local WHS/OHS requirements vary, so check current material from the regulator in your state or territory. A national Model Code can provide general orientation, but it is not automatically applicable law in every jurisdiction.
Main formwork hazards
Formwork includes the face that contains wet concrete and the framing, bracing and supports that keep it stable while the concrete gains strength. Conditions change quickly as components are erected, loaded, adjusted and removed.
The main hazards include:
- collapse from unsuitable ground, missing bracing, poor connections, overloading or unauthorised changes
- falls from leading edges, incomplete decks, ladders and open penetrations
- falling tools, materials, formwork components or wet concrete
- contact with cranes, concrete pumps, delivery vehicles and other powered plant
- electrical services and overhead or underground powerlines
- manual handling of heavy, awkward or repetitive components
- impalement on reinforcement, poor access, slips, trips, wind and rain.
Small warning signs matter because local movement or damage can affect a much larger part of the system.
What to sort out before work starts
The formwork should be designed before it is erected. Suitable calculations, analysis, testing or examination should confirm that the assembly can support expected loads and is fit for its intended use.
Before work begins, the project team should agree on:
- current drawings, manufacturer instructions and the erection, loading, pour and stripping sequence
- who controls the design and who may approve a change
- ground or supporting-slab capacity, footings, props, bearers, bracing and ties
- safe access, work platforms, edge protection and penetration covers at every stage
- lifting arrangements, storage areas, exclusion zones and plant movements
- loading limits, pour rate and where materials may be placed
- inspection, certification, handover and stripping hold points
- emergency arrangements and communication during the pour
- consultation between the builder, formworker, concreter, pump operator and affected trades.
Workers need information, training and instruction that cover the work, its risks and the controls being used. A pre-start should make the day’s sequence, restricted areas and changes clear.
What good controls and documents usually cover
Use the hierarchy of control. Eliminate exposure where reasonably practicable, then prioritise engineering and isolation measures before administrative controls and PPE.
For formwork, higher-order controls commonly include a system designed for the actual loads and site conditions, stable foundations, correctly installed bracing and proprietary connections, edge protection, secured penetration covers, safe access platforms, mechanical lifting and physical separation from mobile plant and overhead work.
Documents usually support those controls through:
- design drawings, specifications and supplier instructions
- a project risk assessment and relevant SWMS
- an agreed erection, alteration, concrete placement and dismantling sequence
- inspection, pre-load certification and deck handover records
- lift, traffic, exclusion-zone and emergency arrangements
- pre-start, toolbox, training, maintenance and corrective-action records.
Documents and drawings should match the system on site. If a component, dimension, load, foundation condition or sequence changes, pause and have the change reviewed through the agreed process before continuing.
When a SWMS or another site document applies
A SWMS is required before high risk construction work starts. Formwork commonly involves high-risk situations under local rules, including serious fall exposure, temporary support needed to prevent collapse, work near energised services or work in areas where powered mobile plant is moving.
The SWMS should identify the high risk construction work, the health and safety risks, the controls, and how those controls will be implemented, monitored and reviewed. Develop it with the workers doing the work, keep it accessible and use it in pre-starts, supervision and change management.
Other site-specific documents may also be needed, such as engineered design information, a designer safety report, lift or traffic plans, emergency procedures, inspection certificates, deck handovers or stripping authorisation. The right set depends on the system, work stage, contract, site and jurisdiction.
Practical warning signs
Stop, keep people clear and escalate when you see:
- settlement, movement, unusual deflection, cracking or unexpected noises
- missing, damaged, loose or displaced props, braces, ties, pins or connections
- components mixed, substituted or altered without review
- loads placed outside nominated areas or above stated limits
- incomplete edges, unsecured penetration covers or unsafe access
- plant striking or crowding supports, or workers entering exclusion zones
- a pour moving faster or differently from the agreed sequence
- stripping starting without the required inspection or authorisation.
A competent person should inspect completed formwork before it is loaded with concrete. During placement, monitor the assembly and stop the pour for adjustments or repairs when directed by the competent person.
