It has been cited for its apparent shortcomings in assuring, to a degree that is fairly practicable, that individuals outside of its direct employment were not unduly subjected to health and safety risks.
The subject of these allegations took their own life while receiving care within the hospital’s perinatal mental health unit, a deeply regrettable incident that occurred in November 2023.
